Requirements to Play

  1. You must be registered on the registermyathlete.com website. 
    Select Nevada as your state and find Coleville High School to begin your registration.
  2. You must have a physical (a physical is good for two years).
  3. You must complete the following forms:
    1. Medical Questionnaire
    2. Consent for Treatment
    3. Consent for Drug Testing
    4. Concussion Information
    5. ESUSD Athletic Policy
    6. Proof of Insurance (football only)
