Requirement to Play

1.  You must be registered on the registermyathlete.com website.  

     Select Nevada as your state and find Coleville High School to begin your registration.

2.  You must have a physical (a physical is good for two years).

3.  You must complete the following forms:

     a) Medical Questionnaire 

     b) Consent for Treatment

     c) Consent for Drug Testing

     d) Concussion Information

     e) ESUSD Athletic Policy

     e) Proof of Insurance (football only)
